What information do we process?
To request an appointment for physiotherapy services, you must have a username and password and complete a reservation form.
In addition, a contact form is available for Quirónprevención.
The categories of personal data that we process are those listed below:
- Physiotherapy appointment booking: Identification and contact details (name and surname, telephone number, email address).
- Website access credentials: Username-email address, password.
- “Contact us” form: Identification and contact information (name and surname, telephone number, email address).
The data we request from you through the aforementioned forms are for identification purposes and are the minimum necessary to be able to provide you with the services you request. The data that must be completed are specified in the form itself, and your refusal to provide them will mean that we will not be able to process your request.
How did we obtain your personal data?
The personal data obtained has been provided directly by you when contracting the physiotherapy service or through the different forms available on the website.
For what purpose do we process your personal data?
Personal data may be processed by Quirónprevención for the following purposes:
- Physiotherapy appointment booking and access credentials: In order to manage the appointment for the physiotherapy service contracted with Quirónprevención.
- “Contact us” form: The personal data provided through this form will be processed for the sole purpose of responding to your request for information, questions and/or queries.
Quirónprevención will process the information confidentially and exclusively for specific, explicit and legitimate purposes and will not subsequently process it in a manner incompatible with said purposes. When the subsequent processing of personal data is planned for a purpose other than that for which it was collected, the interested party will be provided, prior to said subsequent processing, with information about that other purpose and any relevant additional information.
What is the legitimacy for the processing of your personal data?
The legal basis that legitimizes the processing of your personal data when booking a physiotherapy appointment and providing access credentials is Quironprevención's compliance with the physiotherapy service contract in which you are an interested party.
The legal basis that legitimizes the processing of your data in the “Contacts” form is the informed, express, free and unequivocal consent that you provide us when you check the box “I accept the Privacy Policy”.
The data we request from you is appropriate, pertinent and strictly necessary and you are under no obligation to provide it to us.
How long will we retain your personal data?
The personal data you provide us will be kept for the minimum time necessary for the correct management of the appointment for the physiotherapy service offered and the attention to your request for information through the “Contact us” form, as well as to meet the responsibilities that may arise from it and any other legal requirement.
To which recipients will your data be communicated?
Quirónprevención will not transfer the data you provide to third parties, except under legal obligation or with your express consent.
What are your rights when you provide us with your data?
Any person has the right to obtain confirmation as to whether or not Quirónprevención is processing personal data concerning him or her. The data protection rights that you can exercise are:
- Right of access, rectification or deletion: You have the right to access your personal data, as well as to request the rectification of inaccurate data or, where appropriate, request its deletion when, among other reasons, the data is no longer necessary for the purposes for which it was collected.

You may exercise your personal data protection rights by sending a written communication to the registered office of Quirón Prevención, SLU, at C/ Agustín de Betancourt 25, 28003 MADRID, indicating the reference “DATA PROTECTION” or to the email address provided for this purpose, protecciondedatos@quironprevencion.com, including in both cases your NIF number, and indicating your name and surname, the request in which the application is specified, address for notifications, date and signature. You may also exercise your rights through legal representation, in which case you must provide an authentic document proving the representation of the third party.
If you request it, we will provide you with the forms in which you can exercise these rights, indicating which right you wish to exercise.
Can I withdraw consent?
You have the right to withdraw consent at any time, without affecting the lawfulness of processing based on consent prior to its withdrawal.
Where can I file a complaint if I feel that my data is not being processed correctly?
If you consider that your data is not being processed correctly, you can direct your complaints to the Data Protection Officer of Quirón Prevención (dpo@quironprevencion.com) or to the control authority, which in the national territory is the Spanish Data Protection Agency, either through the electronic headquarters of its web portal (www.agpd.es), or by writing to its postal address (C/Jorge Juan, 6, 28001-Madrid).

In order to safeguard the security of your personal data, we inform you that Quirón Prevención, SLU has adopted all the necessary technical and organizational measures to guarantee the security of the personal data supplied, including -among others-:
